We are very happy to welcome you to our dental practice and want you to know that we appreciate the chance to take care of you and your family.  Our office is focused on providing you with high quality, gentle dental care. 

Before your appointment time, the forms below will need to be filled out. Feel free to fill them out before you arrive. Otherwise, please arrive at our office about 10 minutes before your scheduled appointment time so you can fill out the paperwork beforehand.

If you have dental insurance and are able to do so, please call our office and provide us with your insurance information 1-2 days before your first appointment. That way we can check your insurance eligibility beforehand. When you arrive at your appointment, please bring any dental insurance cards with you and any additional insurance information if you have any.
